Apply for Unite for HER’s Wellness Programs

Unite for HER is honored to offer two wellness programs: the Empowered Living Program and  the Wellness Passport Program.

These programs are designed for those who have experienced breast or ovarian cancer and are interested in learning more about support resources.

Program acceptance into our Wellness Passport Program is dependent on funding availability.

All programs are free for breast cancer and ovarian cancer patients in the United States.

How to Apply

  • Click on “Apply” button
  • Select the green “Sign Up” button
  • Read the program information and click the pink “Proceed” button
  • Fill in your personal information, then click “Next”
  • Fill in your Delivery Information/Address, then click “Next”
  • Fill in your Diagnosis information, then click “Next”
  • Review the Release Forms and Terms & Conditions, then click “Submit”
  • Click “Proceed” to confirm your submission

 

Note:  You will not be able to sign in to the Unite for HER Member Portal until your account is activated by UFH staff. 

Your Wellness Program Manager will contact you within two weeks of your application to provide an update on your application status and let you know which program you qualify for.

Unite for HER wellness program member

We provide access to education and resources to support individuals diagnosed with breast or ovarian cancer. These services include, but are not limited to, 12 LIVE Speaker Series, 12 LIVE Ask the Experts, Weekly Meditation, HER Video Library, 48 LIVE Cooking webinars, Nutrition support, Fitness programs, Integrative Therapies, Healthy Beauty, and more!

All services and resources are funded by Unite for HER.
